Community Healthcare Network (CHN) is a nonprofit organization that provides health care to low-income and uninsured New Yorkers. CHN aims to provide access to quality, culturally-competent community-based primary care, mental health, and social services for diverse populations in underserved communities throughout New York City.
CHN is made up of 14 federally qualified health centers throughout Brooklyn, the Bronx, Queens, and Manhattan, along with a fleet of mobile vans that bring health services to people in need. We provide judgment-free, high-quality healthcare, without regard to race, religion, orientation, gender identity, immigration status or ability to pay. We turn no one away.
Our Crown Heights Health Center located at 1167 Nostrand Ave in Brooklyn has partnered with Live Light, Live Right Program to help treat children who struggle with obesity and the health problems it causes. We focus on the specific needs of each child and family. We then create a treatment plan based on their needs. We offer great care that helps to lower health risks through medical intervention, nutrition and behavior change counseling. Our team is deeply committed to our patients and strives to empower them to take control of their health.
